2. HOME MODIFICATION SERVICE OVERVIEW

Home Modification Services include permanent, medically necessary physical changes to a client’s home that enhance safety, accessibility, and independence. These modifications are based on the individual’s assessed needs and must directly relate to the individual's disability or health condition.

Approved providers may deliver:

• Wheelchair ramps and widened doorways
• Grab bars, roll-in showers, and accessible bathroom adaptations
• Stair lifts, transfer equipment, and handrails
• Kitchen and bedroom access modifications
• Fire safety systems for individuals with sensory impairments
• Non-slip flooring and threshold elimination

All projects must be pre-approved and must meet applicable housing codes and waiver-specific standards.

3. LICENSING & PROVIDER APPROVAL REQUIREMENTS

Prerequisites:

• Register your business with the Alabama Secretary of State
• Obtain an EIN from the IRS and a Type 2 NPI
• Demonstrate relevant construction/contracting experience and certifications
• Secure appropriate Alabama contractor licensure (if performing structural work)
• Enroll as a Medicaid Waiver provider under the appropriate waiver(s)
• Maintain liability and workers’ compensation insurance
• Use HIPAA-compliant systems for client files and documentation
