Role: Manages provider enrollment, billing, and reimbursement for Meal Delivery Services

Agency: Centers for Medicare & Medicaid Services (CMS)**
Role: Provides federal oversight to ensure that HCBS waiver services, including meal delivery, meet Medicaid compliance and support independence

2. MEAL DELIVERY SERVICE OVERVIEW

Meal Delivery Services ensure that individuals with IDD receive healthy, balanced meals delivered to their residence on a regular basis, based on their person-centered service plan. Services must align with the individual’s dietary restrictions, preferences, and nutritional needs. Approved providers may deliver:

• Daily or scheduled home-delivered meals
• Special dietary meals (low sodium, diabetic, renal-friendly, soft/pureed textures, etc.)
• Individually packaged, ready-to-eat or heat-and-serve meals
• Meals that meet minimum USDA nutritional guidelines
• Delivery tracking and service verification

Coordination with caregivers or support teams for meal planning

All meals must be delivered safely, timely, and in accordance with state health codes and individual dietary requirements.

3. LICENSING & PROVIDER APPROVAL REQUIREMENTS

Prerequisites:

• Register business with the Alabama Secretary of State
• Obtain EIN from the IRS and NPI (Type 2)
• Maintain a licensed commercial kitchen or contract with a licensed food preparation vendor
• Acquire food service permits from the local health department
• Maintain liability insurance, delivery tracking systems, and food safety procedures
• Implement HIPAA-compliant documentation and delivery protocols

4. ADMH PROVIDER ENROLLMENT PROCESS

Initial Interest and Application:

• Request the Meal Delivery Services Provider Application Packet from ADMH
• Submit a service plan detailing meal preparation, delivery schedule, dietary accommodations, and food safety protocols
• Include menus, delivery logs, and examples of client communication and consent forms

Background & Credentialing:

• Background checks for all staff, especially those involved in food handling or delivery
• Proof of food handler certifications, health inspections, and kitchen compliance
• Documentation of training in food safety, infection control, and consumer interaction

Program Readiness Review:

• Review of menus, nutritional analysis, and storage/transport procedures
• Delivery route planning and verification of service tracking tools
• Emergency food delivery and consumer complaint resolution procedures

Approval & Medicaid Enrollment:

• Once approved by ADMH, enroll with Alabama Medicaid as a Meal Delivery Services provider
• Add applicable billing codes for home-delivered meals to Medicaid provider profile

5. REQUIRED DOCUMENTATION

• Articles of Incorporation or LLC Certificate
• IRS EIN Letter
• NPI confirmation
• Local health department food permit
• Commercial kitchen lease or contract with a licensed meal vendor

ADMH-Compliant Policy & Procedure Manual including:

• Meal planning, dietary accommodation, and portion control standards
• Client intake and delivery consent forms
• Delivery logs and missed meal procedures
• Confidentiality and HIPAA compliance policies
• Emergency preparedness for meal interruptions
• Staff training logs and vehicle sanitation checklists
• Signed consumer meal consent forms and dietary preference sheets

6. STAFFING REQUIREMENTS

Role: Program Manager / Nutrition Coordinator
Requirements: Background in nutrition, food service, or healthcare; experience managing food delivery operations

Role: Food Service Staff / Kitchen Personnel
Requirements: Food handler certification; training in meal prep, sanitation, and portion control

Role: Delivery Driver
Requirements: Valid driver’s license; clean background check; training in client interaction, delivery protocol, and confidentiality

All staff must complete:

• ADMH orientation and HCBS waiver training
• Annual training in food safety, infection control, ethics, and client confidentiality
• Emergency procedures and safe food handling protocols

7. MEDICAID WAIVER SERVICES

The following Alabama Medicaid Waivers authorize Meal Delivery Services:

• ID Waiver (Intellectual Disabilities Waiver)
• LAH Waiver (Living at Home Waiver)
• State of Alabama Community Waiver Program

Approved providers may deliver:

• Regular, home-delivered meals tailored to individual needs
• Documentation of delivery, nutrition, and client satisfaction
• Coordination with care teams to ensure health and wellness outcomes
