3. Licensing or Certification

No medical license is required. Providers must secure Initial Provider Certification from the Wyoming Department of Health, Developmental Disabilities Section, renewed every 3 years.

4. Responsible State Agency

• Wyoming Department of Health (WDH): Oversees waiver compliance and provider certification
• Wyoming Secretary of State: Handles corporate entity registrations

5. Application Process

• Register business structure with the Wyoming Secretary of State
• Obtain an IRS EIN and a Type 2 Organization NPI
• Submit a provider certification application via the state portal (IMPROV)
• Clear fingerprint-based background checks through the DCI and DFS
• Enroll with Wyoming Medicaid to finalize the provider billing agreement

7. Timeline for Approval

The processing pipeline for business registration, background screenings, state reviews, and final Medicaid portal activation typically takes 2 to 3 months.

8. Pre-Application Process

Form an LLC or Corporation, obtain an EIN, secure a Type 2 NPI mapped to community support taxonomies, and map out local community assets before applying.

9. Pre-Application Training

Staff must complete mandatory online WDH modules and provide training summaries (Form EX24) for:

• Rights of Participants & HCBS Standards
• Notification of Incident Process (IRIS)
• Abuse, Neglect, and Exploitation (ANE) Prevention
• In-person certified CPR and First Aid

10. Additional Notes

• Non-Duplication: Cannot be billed simultaneously with Day Habilitation or Supported Employment
• Goal Tracking: Every outing must directly map to a distinct goal within the participant's Individualized Plan of Care (IPC)
• Conflict-Free: Case managers cannot have a financial interest in the service-providing agency
