3. Licensing or Certification

An agency does not require a specialized medical facility license, however, formal Medicaid HCBS Provider Certification from the Wyoming Department of Health is mandatory. Certifications must be updated and re-verified every 5 years.

Before starting the application, the provider must legally establish their corporate structure with the Wyoming Secretary of State, secure an EIN, and obtain a Type 2 NPI mapped to relevant home care or social service taxonomies.

Agency administrators and homemaker staff must successfully complete the mandatory initial provider training sequence hosted by the state and maintain verified documentation (Form EX24).

• Under Chapter 42 rules, homemaker providers are typically restricted to a maximum of three hours of service per week per participant unless an explicit exception is approved by the state
