5. Application Process

• Register the corporate entity with the Wyoming Secretary of State

8. Pre-Application Process

Prospective providers must incorporate with the Secretary of State and secure an EIN and NPI. Agencies must also set up an Electronic Visit Verification compliant tracking mechanism, as Wyoming mandates automated visit verification for all home-based personal care hours.

9. Pre-Application Training

Agency administrators and care aides must complete mandatory online modules through the WDH learning management platform. Verification forms must be submitted.

• Providers must use an approved Electronic Visit Verification system to log location, clock-in/out times, and caregiver identity for all home visits
• Care aides are strictly limited to verbal or visual reminders and opening pre-packaged pill organizers. Direct administration is prohibited without a clinical license
• Medically complex participants or cases involving delegated health tasks may trigger a protocol requiring periodic supervisory visits by a Wyoming-licensed Registered Nurse 
• Shift logs must explicitly match the participant's authorized Individualized Plan of Care goals to withstand state financial audits and prevent funding clawbacks
