Prerequisites: 

• Register your business with the Arizona Corporation Commission
• Obtain EIN from the IRS and NPI (Type 2)
• Apply through the Qualified Vendor Application and Directory System (QVADS) (if serving DDD members)
• Meet local food safety and handling regulations (Maricopa County or local jurisdiction)
• Maintain kitchen inspection records, food handler certifications, and delivery logs
• Hire or contract with a registered dietitian (for menu development and compliance)

4. DDD QUALIFIED VENDOR ENROLLMENT PROCESS

Step 1: Orientation and Application

• Attend DDD Qualified Vendor orientation
• Submit QVADS application, including service plan, dietary protocols, and delivery systems

Step 2: Documentation Submission

• Include kitchen licenses, sample menus, and food safety training records
• Submit intake and tracking forms for delivery and consumer satisfaction

Step 3: Contract Execution

• Once approved, provider signs Qualified Vendor Agreement
• Begin receiving meal referrals from support coordinators

5. REQUIRED DOCUMENTATION

• Articles of Incorporation or Business License
• IRS EIN Letter
• NPI confirmation
• Food establishment license or kitchen inspection approval
• QVADS registration (if applicable)

Policy & procedure manual including: Menu planning and diet compliance guidelines:

• Meal prep, packaging, and delivery protocols
• Emergency meal planning and delivery backup procedures
• HIPAA compliance, client consent, and intake forms
• Food handler training and supervision records
• Complaint resolution, allergen risk procedures, and incident reporting
• Weekly delivery logs and nutritional monitoring tools

6. STAFFING REQUIREMENTS

Role: Nutrition Program Coordinator / Supervisor Requirements: Experience in food service, health programs, or meal planning, ServSafe Manager or equivalent

Role: Meal Prep Staff / Drivers Requirements

• Valid food handler card
• Clean driving record and background check
• Knowledge of dietary restrictions and safe food transport

Optional:

• Registered Dietitian (required if offering therapeutic meal plans)
• Training Requirements for All Staff
• Food safety and sanitation
• Special diet handling and allergen awareness
• Client interaction and confidentiality
• Incident reporting and meal tracking

7. MEDICAID WAIVER SERVICES

Meal & Nutrition Services are authorized under

• ALTCS – Division of Developmental Disabilities (DDD) Waiver
• ALTCS – Elderly and Physically Disabled (EPD) Waiver

Approved providers may deliver:

• Meal preparation and home delivery
• Menu planning tailored to medical needs
• Client education (if included in the ISP)
• Tracking of nutritional goals and preferences

8. TIMELINE TO LAUNCH

Phase Business Formation Timeline: 1–2 weeks

Phase Food License Application and Kitchen Setup Timeline: 30–45 days

Phase QVADS Application and Submission Timeline: 60–90 days
